It is the goal of the Performance Improvement Program to enhance the quality of care provided to CHC patients. The PI program has been developed in the effort to help understand and improve health care processes and outcomes.
The CHC PI program is a collaborative and interdisciplinary effort that ensures that a systematic, organization-wide approach is made toward process design, performance measurement, assessment, and improvement of processes and outcomes, while affirming the Community Health Center's mission, vision and values.
The Community Health Center has operationalized the PI program with the establishment of the Performance Improvement Quality Team. This team remains accountable to the CHC Board of Directors and to Executive Management while representing patient, support, and operational services. Upon reviewing suggestions for improvement, the Performance Improvement Quality Team is charged with prioritizing concerns for improvement, and determining the need for the establishment of smaller interdepartmental PI teams.

Newly formed PI teams are assigned the task of working together on areas for improvement using the FOCUS-PDCA approach. The Performance Improvement Quality Team will track the smaller P.I. teams progress and evaluate new or improved processes to assure their service design meets basic performance improvement expectations. The Performance Improvement Quality Team is also responsible for the development of an Annual Performance Improvement Assessment and presentation to the Board of Directors.
The Community Health Center recognizes that the Performance Improvement program should not only be used for "performance improvement projects," but is also meant to be woven into the fabric of the ordinary work day as a tool for creating effective and efficient processes. Therefore to further meet its mission of "enhancing the quality of life" of our patients, the Community Health Center has also made a commitment to maintain JCAHO accreditation in both behavioral health and medical services.
