Community Health Center Medical Records

The Community Health Center Department of Information Services provides the following services:

  • Maintains customer service hotline
  • Provide data entry
  • Serve as a HIPAA resource
  • Process and maintain Patient records
  • Provide transcription
  • Maintain confidentiality
  • Process release of information
  • Process mail and photocopying
  • Provide statistics
  • Train all staff in customer relations program

Answer subpoenas and court orders in compliance with Title 42 CFR Part 2 and Title 45 HIPAA, Part 160 and 164

The Community Health Center is governed by Federal Law Title 42 CFR, Part 2 and Title 45 HIPAA, Part 160 and 164. Congress enacted this legislation so that people would seek treatment for alcohol and chemical addictions without fear of discovery. The Community Health Center will release information only under the following conditions:

Child abuseCourt order
Communication among personnel Medical emergency
Crimes on program premises, or against personnelValid authorization- signed by Patient
Audits, research, and evaluation activitiesRequired by law
