Take Home Dose
  1. New opioid treatment patients will be eligible for take-homes after being on the program for six months, with 90 consecutive days drug-free.


  2. Take homes are not a patient's right, but rather an earned privilege.


  3. Other eligibility criteria, as mandated by the State of Ohio are as follows:
    • Demonstrated progress towards rehabilitation.
    • Absence of recent criminal activity or involvement with probation/parole.
    • Absence of serious behavioral or emotional problems.
    • Employment/school attendance/homemaking, physical disability issues.
    • Compliance with treatment plan requirements including consistent attendance at counseling sessions, dosing, and doctor's appointments.
    • Stable home and social relationships.
    • Safe storage of take-home doses/lockbox.
    • Demonstrated abstinence from all illicit substances including alcohol and non-approved medication for a minimum of 90 days.

  4. Time frames for take-home eligibility are as follows:
    • Patients in the program at least six (6) months with 90 consecutive days of drug/alcohol-free time area eligible to apply for three (3) take-home doses per week, with attendance at the clinic four (4) times per week.
    • Patients in the program at least 12 months and drug-free for six (6) consecutive months may apply for six (6) take-home doses per week, with attendance at the clinic one (1) time per week>
    • Patients in the program at least 24 months and drug-free for 12 consecutive months may apply for 13 take-home doses every two weeks, with attendance at the clinic two (2) times per month.

  5. Other Community Health Center requirements include:
    • Good standing with the billing department. No outstanding balances
    • Completion of the Opioid Agonist Medication Take-Home Contract.
    • If take-home doses are revoked for any reason, the minimum time frame for reinstatement is 30 days. All cases will be handled on an individual basis depending upon the type of infraction and input from the patient's counselor. The Medical Director will make the final decision.
    • Providing a current address and telephone number to the medical department and your counselor.
